What is gastric sleeve surgery?
Gastric sleeve (also known as sleeve gastrectomy) is recognised as the gold standard of weight loss surgery.
During a gastric sleeve procedure, a section of your stomach pouch is removed making your stomach significantly smaller. That means you feel comfortably full after eating a much smaller portion of food. You also feel full for longer, reducing the temptation to snack between meals.
A gastric sleeve operation is usually performed laparoscopically (meaning keyhole surgery). Keyhole surgery uses much smaller incisions, aiding your recovery.
Who might benefit from a gastric sleeve?
Generally speaking, you’re eligible for obesity surgery if:
- Your BMI is over 35
- Your BMI is over 30 and you have a weight-related condition like diabetes, high blood pressure, sleep apnoea or high cholesterol.
What are the pros and cons of a gastric sleeve procedure?
There are several different types of weight loss surgery, each with its own strengths and weaknesses. If you’re considering a gastric sleeve, then it’s important to understand the pros and cons before you decide.
The advantages of a gastric sleeve are:
- It’s recognised as the gold standard of weight loss surgery.

High rates of excess weight loss.
Excess weight refers to the kilos above your ideal body weight.
With a gastric sleeve, you can expect to lose about 40-100% of your excess weight (the average is 85%). - It's a 'set and forget' procedure with no need for regular medical reviews and adjustments like the gastric band.
- In time, you can return to eating more fibrous solid foods such as steak, chicken breast, prawns and bread, which is not possible with the gastric band.
- The gastric sleeve does not cause any changes to food absorption.
- Hospital stay is generally 3 days, predominantly for monitoring.
- Requires vitamin supplementation after the procedure.
- Helps your body to create GLP1 hormone changes which corrects sugar and changes taste sensations.
- Allows your body to create PYY hormone changes which suppresses appetite.
- Helps your body create ghrelin hormone changes – the hormone which creates hunger.
The main disadvantage of a gastric sleeve operation is the risk of leakage from the suture (stitches) line where the section of your stomach was removed. If that happens, you’ll need further surgery to repair the leak and will stay in hospital for monitoring.
That sounds scary but it’s very rare. New technology and improved surgical techniques have addressed this problem, reducing the risk of this complication to a miniscule number of cases. Dr Durmush has performed more than 2600 primary sleeves with a complication rate of less than 0.5%, over 2000 operations with no leak in the last 7 years.
Comparing a gastric sleeve with a gastric band
Research shows that a gastric sleeve consistently achieves greater weight loss than the gastric band and also has significant impacts on the reduction of other obesity-related medical conditions such as diabetes. Patients that proceed with the gastric sleeve lose between 30-40% more weight than patients who undertake the gastric band surgery. Failure rate of gastric band is 70%, complication rate 30%.
What's the difference between a gastric sleeve and a gastric bypass?
A gastric sleeve reduces the size of your stomach but makes no other changes to normal digestion. It’s purely a restrictive surgery. Also, patients do not lose some of the micronutrients and proteins as they do with the gastric bypasses.
A gastric bypass reduces the size of your stomach and removes the first part of your small intestine which changes how your body absorbs food. It’s both a restrictive AND a malabsorptive surgery. Patients are likely to lose micro-nutrients as a result.
